Department: OPAT Clinic - SRMC | FTE: 1.00 Full Time | Shift: Days

About the role

Provide safe, direct patient care utilizing the nursing process in the ambulatory setting. Provide clinical leadership and mentorship to staff, serve as a role model, and deliver comprehensive patient education to patients and family members. Assume relief charge duties when designated, under the direction of the designated shift supervisor. Ensure adherence to hospital and departmental policies and procedures. Patient care assignments may include neonate, pediatric, adolescent, adult, and geriatric age groups.

Responsibilities

  • Follow patient safety-related policies, procedures, and protocols.
  • Demonstrate a proactive approach to patient safety by seeking opportunities to improve policies and processes.
  • Identify and report or correct environmental conditions that may put patients at risk.
  • Assist with or institute emergency measures for sudden, adverse developments in patients.
  • Assess patient conditions, set nursing goals, and utilize appropriate actions to achieve them.
  • Provide and document nursing care, ensuring physician’s orders are carried out.
  • Maintain equipment, monitor supply and medicine requisitions, and maintain stock levels.
  • Record and report pertinent patient observations and reactions to medical staff.
  • Administer medications and perform routine treatments.
  • Assist physicians with special tests, procedures, and complex treatments.
  • Participate in the orientation and evaluation of new personnel.
  • Coordinate patient care with other departments and clinics, and assist team members as needed.
  • Process data, generate reports, and assist in reception and discharge areas.
  • Handle patient calls, triage using documentation, and ensure medical records contain necessary information.
  • Document lab cultures and ensure follow-up on abnormal results.
  • Report potential or actual patient safety concerns, medical errors, or near misses in a timely manner.
  • Encourage patients to actively participate in their care by asking questions and reporting concerns.
  • Adhere to and promote core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice.
  • Serve as a mentor to patients to promote self-management, hope, and empowerment.
  • Provide education for patients and families on preventive care, medical issues, and prescriptive treatments.

Requirements

  • Graduate of a nationally accredited nursing program (Bachelor’s degree in Nursing preferred).
  • 3 years of directly related experience.
  • Bilingual proficiency in English and one of the following: Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or Navajo (preferred).
  • Current CPR for Healthcare/BLS Provider or Professional Rescuers certification (within 30 days of hire).
  • RN licensure in New Mexico or as allowed by reciprocal agreement.

Physical Conditions

  • Heavy Work: Exerting 50 to 100 pounds of force occasionally, 25 to 50 pounds frequently, or 10 to 20 pounds constantly to move objects or people.
  • Moderate exposure to chemicals, dangerous equipment, or hazardous materials.
  • Tuberculosis testing completed upon hire and as required thereafter.
