About the role
The Oncology Nurse Navigator (PONN) coordinates and implements strategies to ensure quality, efficacy, and cost-effectiveness for oncology patients in various settings. They assess patient needs, orient patients to hospital and outpatient settings, and manage the complex aspects of the cancer diagnosis. The PONN collaborates with multidisciplinary teams, communicates effectively, and manages the transition from diagnosis to survivorship and end-of-life care.
Responsibilities
- Coordinates and implements strategies for quality, efficacy, and cost-effectiveness in oncology care.
- Affirms patient needs and provides orientation to hospital and outpatient settings.
- Provides cancer care across the continuum, from diagnosis through treatment and transition to survivorship and end-of-life care.
- Uses the nursing process to educate and resource patients and families for informed decision-making and timely access to quality health and psychosocial care.
- Collaborates with multidisciplinary teams to address patient needs and concerns.
- Manages the complexity of the cancer diagnosis and communicates with all team members.
- Reviews distress screening for referrals and develops and reports outcome metrics to drive program development.
- Supports the community needs assessment of site-specific results and reinforces education regarding diagnosis.
- Connects patients to appropriate resources and develops and implements activities that support COG standards.
- Empowers patients to self-advocate and maintains continuous learning and competency.
- Provides emotional support for patients and advocates on their behalf.
Requirements
- Bachelor of Science in Nursing (BSN)
- Nurse, Registered (RN) license
- BLS certification
- OCN Certification or Nurse Navigator Certification within 2 years of hire
