About the role
The Oncology Navigator at Reid Health System is tasked with navigating patients through their cancer journey, aiming to break down barriers and ensure comprehensive care.
Responsibilities
- Coordinates multiple aspects of cancer patient care within the Reid Health System.
- Provides continuity of care, supporting patients' needs for education, care coordination, and psychosocial support throughout their experience.
- Develops educational programs, formulates policies and procedures, establishes goals, and engages in problem-solving activities.
- Assesses and plans patient care, collaborates with patients, families, and healthcare teams, and modifies or continues the care plan based on patient needs.
- Implements nursing and medical interventions, assists with pre- and post-procedural care, makes appropriate referrals, and provides patient and family teaching to achieve desired outcomes.
- Communicates and documents interventions, patient responses, and referral information according to standards and complies with departmental and organizational changes.
- Schedules appointments for additional diagnostic screenings and procedures, coordinates with multiple physician offices, and ensures appropriate follow-up care.
- Provides one-on-one and group education and support for patients and families.
- Maintains a working knowledge of infection control, risk management, and emergency preparedness plans, participates in performance improvement initiatives, and demonstrates responsiveness to organizational goals and standards.
- Outreach and education to outside medical groups and the community, in conjunction with clinic leadership.
Requirements
- Bachelor of Science in Nursing required.
- Master’s Degree in Nursing preferred.
- At least 3 years of clinical experience with oncology patients.
- Previous patient group teaching and education experience preferred.
Qualifications
- Indiana Nursing License required.
- Certifications in appropriate fields required.
