Key Responsibilities

  • Conduct comprehensive assessments of patients’ physical, psychological, social, and functional status.
  • Coordinate timely appointments and facilitate transitions across care settings.
  • Collaborate with physicians and multidisciplinary teams to develop evidence-based treatment plans.
  • Monitor and adjust care plans based on patient progress and evolving needs.
  • Identify high-risk patients and proactively address barriers to care.
  • Provide education to patients and families on disease processes, treatment expectations, and available resources.
  • Serve as a clinical expert and resource for staff education and internal consultation.
  • Participate in policy development, program planning, and performance improvement activities.

Qualifications

  • Education: Bachelor’s degree in Nursing preferred.
  • Licensure/Certification: Current State of Michigan Registered Nursing License required. Certification in subspecialty (e.g., OCN, CNRN) required. Additional subspecialty certifications preferred.
  • Experience: Minimum 3 years in an acute care setting. At least 1 year in medical, surgical, or radiation oncology required. Case management or patient care leadership experience preferred. 5 years in subspecialty preferred.
