Full-time, regular position with the Department of Veterans Affairs at the Sallisaw Veterans Home.
About the Role
Positions in this job family are responsible for supervising and managing staff, programs, units, or facilities that provide direct or indirect health services. This includes planning, organizing, and integrating various functions to meet program goals and objectives, as well as directing assigned staff.
Responsibilities
- Assist MDS personnel in completing MDS and Care Plans.
- Ensure new MDS/Care Plans are initiated and completed for new admissions, quarterly reviews, annual assessments, and significant changes within the time frame required by RAI/MDS 3.0.
- Perform weekly interviews and assessments for residents and document accurately in PCC.
- Gather data using PCC documentation and contribute to ongoing assessments.
- Complete Initial, Annual, significant change, and Quarterly MDS assessments in Point Click Care under the supervision of the DON and/or ADON.
- Assist with the calendar and IDCP agenda for residents due for initial, annual, quarterly, and/or significant change assessments, and distribute the agenda to all disciplines.
- Evaluate all residents re-admitted from the hospital for changes in status.
- Post a 7-day assessment list of residents on the IDCP agenda.
- Attend weekly IDCP meetings with other disciplines to discuss residents’ plans of care.
- Report any problems with other departments concerning MDS assessment completion to the DON and/or ADON promptly.
- Stay updated on all MDS changes and regulations by attending MDS 3.0 updates and in-services, and keep administration, supervisors, and staff informed of changes.
- Attend all required meetings regarding resident changes.
- Complete 30-day reviews on all residents with restraints and present in IDCP on the first Thursday of each week.
- Update Care Plans for all residents initially, quarterly, and annually.
- Supervise PCAs who participate in IDCP.
- Evaluate employee performance within designated time frames through periodic and annual reviews.
- Initiate corrective discipline steps when necessary.
- Assess staff training needs and provide 1:1 training and/or alert the In-Service educator.
- Follow, interpret, and enforce facility policies and procedures.
- Participate in assessing and planning care for residents, including behavior management goals and interventions.
- Perform Registered Nurse duties on units as required by staffing availability.
- Plan, organize, and direct health services for an assigned program, unit, facility, or geographical area to ensure quality care and consistency with standards.
- Select, mentor, motivate, and evaluate staff; establish and track quality improvement indicators.
- Develop or implement policies and procedures to ensure efficient and effective delivery of health services.
- Participate in the development of long-range plans for health care programs; interpret and communicate agency and departmental policies.
- Receive, analyze, and evaluate reports and other information to coordinate quality improvement activities.
- Develop budget estimates and monitor expenditures, purchases, and other actions to ensure compliance with budget guidelines.
- Perform other related duties as assigned.
- Subject to on-call duty.
Requirements
- Valid permanent Oklahoma license as a Registered Nurse, as approved by the Oklahoma Board of Nursing, or a Registered Nurse License recognized by the Enhanced Nurse Licensure Compact (eNLC).
- Three years of professional nursing experience, one year of which must have been in a supervisory capacity.
- Alternatively, a bachelor’s degree in nursing and two years of professional nursing experience, one year of which must have been in a supervisory capacity.
- Alternatively, a master’s degree and one year of professional nursing experience in a supervisory capacity.

Special Requirements
- Subject to on-call duty.
- Must be willing and able to fulfill all job-related travel normally associated with this position.
- Upon appointment, incumbents may be required to complete certification in one of the following Department-sponsored training programs: Certified Behavioral Health Case Manager – Adult; Certified Behavioral Health Case Manager – Child, Adolescent and Family; or Certified Behavioral Health Case Manager – Dual Certification.
- Upon a conditional offer of employment, applicants must submit to a physical examination and meet the requirements of occasionally lifting/carrying 50 pounds and pushing/pulling a maximum force of 25 pounds.
Pay
$43.00 USD hourly.
