RN Nurse Manager, Home Care

About the role

BAYADA HOME HEALTH CARE is seeking a Registered Nurse Clinical Manager for their Assistive Care State Programs office in Clifton, NJ. This full-time role offers a Monday-Friday daytime schedule, a competitive salary, and a sign-on bonus.

Responsibilities

• Conduct in-home client assessments and ongoing reassessments as a Registered Nurse Clinical Manager
• Develop, review, and update individualized plans of care as the assigned Registered Nurse leader
• Provide clinical oversight for clients receiving services through our Assistive Care State Programs office
• Complete required Registered Nurse documentation, clinical notes, and care coordination activities
• Partner with the office team to help ensure services remain aligned with care plans, client needs, and state program requirements
• Help create a smooth, positive experience for clients who want to remain safe and comfortable at home

Requirements

• Current Registered Nurse license
• At least 2 years of RN experience with a bachelor's degree (3 years with an associate degree)
• Strong clinical judgment, communication, and relationship-building skills
• Ability to complete assessments, develop care plans, and manage clinical follow-up with confidence
• Passion for helping clients safely remain at home with comfort, independence, and dignity

Qualifications

• Home care experience
• Experience in case management, care coordination, or community-based nursing
• Experience supporting caregivers or collaborating with interdisciplinary office teams
