What You'll Do
- Lead and coach a multidisciplinary team of nurses and therapists to deliver outstanding patient-centered care through a collaborative approach
- Foster a culture of teamwork, accountability, clinical excellence, and continuous learning
- Conduct clinical assessments, supervisory visits, and field support throughout Lehigh and Carbon County
- Oversee the implementation of home health services
- Mentor clinicians through complex patient care situations while promoting sound clinical decision-making
- Oversee new hire orientation, onboarding, and ongoing professional development
- Complete competency evaluations for both new and tenured clinical staff
- Review, monitor, and coach clinicians on timely and accurate clinical documentation within Homecare Homebase (HCHB)
- Monitor quality outcomes, regulatory compliance, and performance metrics while supporting continuous quality improvement initiatives using Mosai, an integrated AI platform
- Partner with office leadership to improve clinical outcomes, patient satisfaction, and operational performance
- Build strong relationships with clients, families, physicians, referral partners, and interdisciplinary team members
- Recognize, encourage, and support clinician growth while fostering employee engagement and retention
What We're Looking For
- Current Registered Nurse (RN) license
- Minimum of two years of RN experience
- At least one year of Home Health and OASIS experience required
- Acute care, medical-surgical, rehabilitation, or sub-acute nursing experience preferred
- Strong clinical judgment and critical thinking skills
- Excellent communication, coaching, and leadership abilities
- Highly organized with the ability to prioritize multiple responsibilities in a fast-paced environment
- Passion for mentoring clinicians and delivering exceptional patient care
