RN / Nurse Care Coordinator at UW Medical Center-Montlake

About the role

The UW Medical Center-Montlake is seeking a Nurse Care Coordinator to join their interdisciplinary team. This position focuses on care coordination for transplant, orthopedic, and medical-surgical patients.

Responsibilities

• Identification and monitoring of high medical needs patients to ensure timely completion of consults, procedures, and tests, and ongoing team communication.
• Facilitating coordination between providers across the continuum of care.
• Leading medical team and nursing huddles to address medical complications and system or team obstacles.
• Aiding in the ordering of complex medical equipment and ensuring safe transitions of care.
• Developing and monitoring outcome measures related to care coordination and staff and patient satisfaction with discharge planning.
• Providing ongoing consultation and education to staff on improving patient flow within the UWMC system.

Requirements

• Master's Degree in Nursing or related field.
• Three years of professional nursing experience.
• Experience in Case Management is preferred.
• Able to communicate effectively in English, both verbally and in writing.
• Effective interpersonal and leadership skills.
• Ability to access and document information in the electronic medical record.
• Current Washington State RN licensure at time of hire, and current BLS.

Qualifications

Master's Degree in Nursing or related field. Applicable experience may substitute for education.

Pay Range Minimum: $46.97 hourly
Pay Range Maximum: $86.17 hourly
