About the role
The Social Work and Care Coordination department at UW Medical Center - Montlake is seeking a Cardiac Nurse Care Coordinator to support coordinated patient flow during an inpatient stay, leading to a safe and timely discharge.
Responsibilities
- Identification and monitoring of high medical needs patients to ensure timely completion of consults, procedures, and tests.
- Ongoing team communication and facilitation of coordination between providers across the continuum of care.
- Lead medical team and nursing huddles to address medical complications and system or team obstacles.
- Assist with ordering complex medical equipment and ensuring safe transitions of care.
- Develop and monitor outcome measures related to care coordination and staff/patient satisfaction with discharge planning.
- Provide ongoing consultation and education to staff on improving patient flow within the UWMC system.
Requirements
- Master's degree in Nursing or related field.
- Three years of professional nursing experience, preferably in Cardiac.
- Experience in Case Management strongly preferred.
- Able to communicate effectively in English, both verbally and in writing.
- Effective interpersonal skills, leadership skills, and clinical expertise.
- Ability to access information and document in electronic medical record.
- Current Washington State RN licensure at time of hire, and current BLS.

Compensation, Benefits and Position Details
Pay Range: Minimum $46.97 hourly, Maximum $86.17 hourly. Additional compensation includes BSN (or higher degree) premium of $1.00/hour, certification (specialty certs) premium of $1.25/hour, and $5000 relocation assistance (for eligible new employees).
