About the role
The Sanford Health pediatric specialty clinic is seeking an RN navigator to help coordinate care for patients and families. This role focuses on the Pediatric Infectious Disease department, providing specialized care and support.
Responsibilities
- Coordinate care to ensure patients and families receive necessary treatment, education, and resources.
- Collaborate with healthcare teams and other professionals to streamline care processes.
- Support patients and families by addressing barriers to care.
- Evaluate patient outcomes using clinical, financial, and utilization data.
- Work in various settings, including clinics, hospitals, and community outreach programs.
- Communicate effectively with providers, insurance companies, and other stakeholders.
- Facilitate group meetings and multidisciplinary teams to improve patient care.
- Implement best practices and problem-solve to enhance patient care.
Requirements
- Bachelor's degree in Nursing preferred, or enrolled in a Bachelor’s degree program within one year of hire.
- Two years of clinical experience preferred.
- Current RN license with the State Board of Nursing.
- Required to obtain and maintain department-specific competencies and certifications.
Qualifications
- Graduate from an accredited nursing program.
- Knowledge of age-related patient care protocols for pediatric, adolescent, adult, and geriatric patients.
- Strong communication and interpersonal skills.
