About the Role
Ensures timely and efficient patient referrals and transition throughout the continuum of care from diagnosis to treatment to survivorship. Responsible for the management of resources and services through interdisciplinary collaboration to achieve optimal, contiguous patient outcomes for patients.
Reports to: RN/Nurse Manager
Responsibilities
- Manage patient referrals and transitions across the care continuum
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Oversee resources and services for patient care
Requirements
- Associate degree in nursing (required)
- Bachelor's degree in nursing (preferred)
- Florida registered nurse (RN) license (required)
- Specialty Certification (e.g., Oncology) (preferred)
Qualifications
- Two (2) years of clinical care, case management, or navigation experience (required)
- Five (5) years of related specialty care experience (preferred)
