About the role
The Description Navigator RN at Providence St. Joseph Hospital Orange in Orange, CA is a Full Time position working 8-hour, Day shifts.
Responsibilities
- Supports the Structural Heart Program by coordinating care across the patient journey for procedures including TAVR, MitraClip, LAAO, valve interventions, and research trials.
- Referral screening.
- Patient assessment.
- Diagnostic coordination.
- Clinic scheduling.
- Chart preparation.
- Patient education.
- Care plan documentation.
- Follow-up coordination.
- Participation in weekly structural heart conferences.
- Communication with referring providers.
- Oversight of program registries.
Requirements
- Bachelor's Degree - Nursing.
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 3 years - Cardiovascular experience.
Qualifications
- Master's Degree - Nursing.
- Nurse navigator experience.
