Job Summary
You will provide expert nursing guidance and support to patients as they navigate complex healthcare journeys. Every day you will assess patient needs, educate on treatment options, coordinate care across multiple providers, and facilitate access to resources, acting as a central point of contact.
Responsibilities
- Assess patient needs and provide clinical resources such as referrals for second opinions, treatment or testing, and reliable sources of information.
- Educate patients about their diseases, treatment, and side effects of treatment and how to manage them.
- Assess patients for psychosocial distress and refer to a medical social worker or counselor if more intensive support is needed.
- Answer questions as appropriate and serve as a liaison to the physician for additional questions and concerns.
- Address tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, and much more. Refer patients to the appropriate contact, such as a social worker.
- Ensure any health information provided is accessible: this may include securing documents printed in the patient’s own language or arranging for a certified medical interpretation, either in-person or by phone/video conference.
Requirements
- Required: 3 years clinical nursing experience and Registered Nurse (RN).
- Preferred: Bachelors of Nursing (BSN).
- If working in Oncology, preference for oncology certified nurse (OCN).

$33.51 - $48.58 /hour
