Job Summary
You will provide expert nursing guidance and support to patients as they navigate complex healthcare journeys. Every day you will assess patient needs, educate on treatment options, coordinate care across multiple providers, and facilitate access to resources, acting as a central point of contact.
Responsibilities
- Assess patient needs and educate on treatment options
- Coordinate care across multiple providers
- Facilitate access to resources
- Oversee the disease registry database
- Provide oversight or conduct pre-visit chart reviews of patients
- Work with patients and families on self-management support
- Coordinate care across the care continuum
- Involvement in quality improvement activities
Requirements
- Graduate of an accredited school of nursing approved by the State Board of Nursing
- Associate’s degree in Nursing
- Registered Nurse (RN) license
- Basic Life Support - CPR certification
- Minimum of three years of recent nursing work experience, preferably including experience in conducting nursing education
- Clinic/physician office experience and background in diabetes education
- Prioritize patient-centered care and compassionate approach
- Bachelor's of Nursing (BSN) preferred

Pay Range
$33.84 - $50.34 /hour
