RN - Lead Care Manager (Full-Time, Day Shift)

About the Role

The case management nurse provides exceptional patient care by collaborating with healthcare teams, developing, implementing, and revising care plans, and acting as a liaison among patients, families, and providers to optimize outcomes. They serve as a consultant on patient-specific issues and coordinate interdisciplinary care, ensuring appropriateness and timeliness of treatments in a diverse community setting.

Responsibilities

• Lead care coordination and monitor clinical progress
• Discuss resource utilization with physicians
• Manage transfers to tertiary centers
• Oversee compliance data collection
• Handle complex cases such as frequent ED visitors and vulnerable populations
• Perform assigned tasks and ensure adherence to safety, vaccination, and organizational policies

Requirements

• Active RN licensure (required)
• BSN (preferred)
• Case management certification (preferred)
• Experience in care management
