RN - Lead Care Manager (Full-Time, Day Shift)

About the role

Provides exceptional patient care by collaborating, developing, implementing, and reporting on the case management program. Acts as a liaison among patients, families, healthcare providers, and the multidisciplinary team to optimize outcomes. Serves as a consultant on specific patient needs and coordinates care to ensure appropriateness, timeliness, and continuity.

Responsibilities

• Leads coordination of patient care, monitoring diagnostic testing, treatments, and procedures.
• Ensures interdisciplinary care plans align with the patient's clinical course and ongoing needs.
• Collaborates with physicians on resource utilization, treatment plans, length of stay, and discharge planning, especially for complex, high-utilizer, or vulnerable patient populations.
• Coordinates patient transfers to tertiary centers, ensuring seamless information exchange for ongoing care.
• Provides oversight for regulatory compliance, manages frequent ED visitors through focused reviews, and performs other related duties.

Qualifications & Skills

• Registered Nurse (RN) licensure in the practice state is required.
• Bachelor's Degree in Nursing (BSN) preferred.
• Experience in care management and case management certification are preferred.
• Strong collaboration and communication skills.
• Ability to coordinate multidisciplinary care and manage complex patient cases.
• Knowledge of regulatory and accreditation standards.
