RN - Lead Care Manager (Full-Time, Day Shift)

About the Role

Provides exceptional patient care by supporting the development, implementation, and reporting of the case management program. Acts as a liaison among patients, families, healthcare providers, and the care team to optimize outcomes and ensure coordinated, appropriate care.

Responsibilities

• Leads patient care coordination within the multidisciplinary team, monitoring care appropriateness and timeliness.
• Ensures care plans align with patients' clinical needs, reviewing diagnostics, treatments, and procedures.
• Collaborates with physicians on resource utilization, treatment plans, and discharge planning, focusing on complex and high-utilizer patients.
• Manages patient transfers to tertiary centers, ensuring seamless information exchange for ongoing care.
• Collects data for regulatory compliance and reviews frequent ED utilizers for quality improvement.
• Performs additional related duties as assigned.

Requirements

• Registered Nurse (RN) licensure in the practice state: Required
• Bachelor's Degree in Nursing (BSN): Preferred
• Experience in care management: Preferred
• Case management certification: Preferred
