RN - Lead Care Manager (Full-Time, Day Shift)

About the role

Provides exemplary patient care by supporting the development, implementation, and reporting of the case management program. Acts as a liaison among patients, families, healthcare providers, and the multidisciplinary team to optimize outcomes.

Responsibilities

• Coordinates patient care, monitors care appropriateness and timeliness, and ensures care plans align with patients' clinical needs.
• Collaborates with physicians on resource utilization, treatment plans, and discharge planning, focusing on complex and high-utilizer patients.
• Manages patient transfers to tertiary centers and oversees data collection for compliance.
• Conducts focus studies on frequent ED visitors and performs other related duties.

Requirements

• Bachelor's Degree in Nursing (preferred).
• RN licensure required.
• Case management certification (preferred).
• Strong coordination, communication, and collaboration skills.
• Experience in care management and working with diverse patient populations.
