About the Role

Demonstrate respect, dignity, kindness, and empathy in each encounter with all patients, families, visitors, and other employees regardless of cultural background. Responsible for providing consultation and coordination of the care of patients requiring wound care assessment, management, and treatment (involves skin, wounds, ostomies, pressure sores, incontinence, and other wound care arenas). Collaborates with nursing staff, medical staff, and other health care providers in establishing and executing policies, procedures, and standards of care for wound care management patients.

Responsibilities

  • Provides patients, families, and staff with the knowledge and skills for the care and management of patients' ostomies, wounds, incontinence, pressure sores, and other skin-related issues.
  • Serves as an educator, resource person, and role model in the formulation and follow-through of the patient’s plan of care as it relates to skin management issues and ostomy care.
  • Consults with nursing and medical staff as requested/needed to evaluate, develop, and recommend a management program for the skin-impaired patient to achieve the most positive outcomes.
  • Conducts, upon request, pre-operative assessments for stoma site selection and counseling for patients scheduled for ostomy surgery.
  • Provides input to the Wound Care Manager regarding revisions of current hospital policies, procedures, and standards based on best practices and current clinical research and practice.
  • Provides, in collaboration with the Wound Care Manager, in-service education programs related to skin care management and ostomy care to staff as requested/needed.
  • Assists physicians and nursing staff in coordinating care of patients during their acute hospital phase and on an outpatient basis.
  • Consults with Home Care on patients with skin care/wound/ostomy problems and issues. Makes home care visits as needed.
  • Assists the hospital with the financial aspects of skin management by reviewing, tracking, and following through on products, specialty beds/equipment, and invoice tracking in consultation and collaboration with the Wound Care Manager.
  • Performs other duties as assigned.

Requirements

  • Graduate from an accredited school of nursing.
  • If eligible, must have a Compact (multi-state) License in their primary state of residence.
  • Licensed to practice nursing in the state where work is being performed.
  • BSN or currently enrolled in BSN program.
  • Meets contact hour requirements for licensure, including all state-required courses.
  • Minimum requirements of 3 years current clinical experience in an acute care facility.
  • Prior experience in skin, ostomy, and wound care clinical practice/management.

Qualifications

Specialized Knowledge

  • Basic knowledge in current skin, ostomy, and wound care practices.
  • Understanding of the organizational structures of the hospital, its policies, and practices.
  • Able to be self-directed and highly motivated.

Desirable Qualifications

  • MSN.
  • Certification as a wound, ostomy, continence nurse (WOCN).
  • Graduate of Enterostomal Nursing Education Program.
  • Advanced knowledge and experience in skin, ostomy, and wound therapy and practices.
  • Ability to interface wound care management into daily practices by staff.
  • Current experience as a wound care nurse or enterostomal therapist.

Schedule

32 scheduled hours.
