RN Hospital Community Liaison - Hospice - Full-Time

About the role

Responsible for accepting Hospice, Home Health, and Home Infusion referrals across various settings including acute care, clinics, nursing homes, and rehabilitation centers. Maintains an active presence, providing excellent customer service to discharge planners, social workers, physicians, and patients. Facilitates timely discharges and supports continuity of care within the health system.

Responsibilities

• Accept Hospice, Home Health, and Home Infusion referrals from acute care, clinics, nursing homes, and rehabilitation centers.

• Facilitate timely discharges and support continuity of care within the health system.

Requirements

• Current registered nurse license.
• At least three years of nursing experience, preferably in hospice, home health, or inpatient settings.

Qualifications

• Preferred certification: Hospice and Palliative Nursing (CHPN).
