Hospital-Community RN Liaison - Full Time

About the role

This position involves accepting Hospice, Home Health, and Home Infusion referrals across various settings such as acute care, clinics, nursing homes, and rehabilitation centers.

Responsibilities

• Maintain presence in these environments
• Provide excellent customer service to discharge planners, social workers, physicians, and patients
• Facilitate timely discharges
• Support continuity of care within the system

Qualifications & Skills

• Registered Nurse license in applicable state required
• Bachelor's degree preferred
• Minimum three years of nursing experience, with hospice and home health experience preferred
• Strong communication, observation, assessment, and documentation skills
• Self-directed, flexible, team-oriented, with basic computer skills and understanding of electronic medical records
• Knowledge of current guidelines, regulatory standards, and clinical practices related to hospice and home services
• Ability to communicate services effectively to physicians and post-acute communities
• Experience in inpatient, community, or hospice nursing is advantageous; certification in Hospice and Palliative Nursing (CHPN) a plus
