Hospital-Community RN Liaison - Home Health - Full-Time

Role: Responsible for accepting Hospice, Home Health, and Home Infusion referrals across various settings, including acute care, clinics, nursing homes, and rehabilitation centers. Maintains a visible presence, delivers excellent customer service to discharge planners, social workers, physicians, and patients, and facilitates timely discharges to support continuity of care within the health system. Key Responsibilities: Manage referrals, communicate services to healthcare providers, assess client suitability for hospice and home services, and ensure accurate documentation. Work collaboratively within a team and support system initiatives for patient care continuity. Qualifications & Skills: Current RN license with a minimum of three years of nursing experience; preferred experience in Home Health or acute inpatient settings. Strong assessment, communication, and documentation skills. Self-directed, flexible, and team-oriented with an understanding of regulatory guidelines and hospice philosophy.
