Hospital-Community RN Liaison - Home Health - Full-Time

About the role

The role involves accepting and managing Hospice, Home Health, and Home Infusion referrals across various settings such as acute care, physician clinics, nursing homes, and rehabilitation centers. It requires maintaining a strong presence in these environments to provide excellent customer service to discharge planners, social workers, physicians, and patients, facilitating timely discharges and ensuring continuity of care.

Qualifications

• Current RN license
• Minimum three years of nursing experience, preferably in Home Health or acute settings
• Bachelor's degree preferred
• Strong communication, observation, assessment, and documentation skills
• Ability to work independently, be flexible, and collaborate effectively

Responsibilities

• Assessing client suitability for Hospice and Home Services
• Communicating service options to physicians and community partners
• Supporting discharge planning and care coordination
