Hospital-Community RN Liaison - Full Time

Role Overview Responsible for accepting Hospice, Home Health, and Home Infusion referrals across various settings such as acute care, physician clinics, nursing homes, and rehabilitation centers. Maintains a strong presence in these environments to provide excellent customer service to discharge planners, social workers, physicians, and patients. Supports timely discharges and promotes continuity of care within the healthcare system. Qualifications & Skills Registered Nurse (RN) license in applicable state; bachelor's degree preferred.Minimum of three years nursing experience; hospice and home health experience preferred.Strong communication, observation, and assessment skills; proficient in documentation.Self-directed, flexible, and team-oriented.Basic computer skills and understanding of Electronic Medical Records preferred. Additional Information Demonstrates understanding of regulatory and clinical guidelines, supports hospice philosophy, and communicates services effectively to physicians and communities. Certification in Hospice and Palliative Nursing is a plus.
