Hospital-Community RN Liaison - Full Time

Position Overview A dynamic role responsible for accepting Hospice, Home Health, and Home Infusion referrals across various settings including acute care, physician clinics, nursing homes, and rehabilitation centers. Maintains an active presence in these settings, providing excellent customer service to discharge planners, social workers, physicians, and patients. Supports timely discharges and promotes continuity of care within the health system. Qualifications & Skills Registered Nurse with current licensure in applicable state; bachelor's degree preferred.At least three years of nursing experience; hospice and home health experience preferred.Strong assessment, observation, and communication skills, with ability to document accurately and work independently.Understanding of regulatory guidelines and supportive of hospice and home services philosophy.Team-oriented, flexible, and self-directed with basic computer and electronic medical record skills.Experience providing hospice or home nursing care is a plus; certification in Hospice and Palliative Nursing (CHPN) preferred.
