RN Hospice Coordinator - West Region

About the Role

Cleveland Clinic’s Business Operations Center serves as a key hub for our Home Care and Hospice operations as well as many administrative and support teams. This collaborative workplace supports a wide range of caregivers, clinical professionals, and operational teams. While many positions are based within the center, numerous home-visiting roles—including case managers, nurses, and therapists—travel throughout the Greater Cleveland and Akron/Canton areas to deliver exceptional care directly to patients in their homes. As a Hospice Coordinator, you will ensure a smooth transition for patients as they move from the hospital to home or a facility, helping provide a full continuum of care through our Center for Connected Care.

Responsibilities

• Act as a member of the interdisciplinary team for assigned functions related to the coordination of services with internal and external referral sources.
• Process inpatient referrals into hospice, ensure continuity of care, and coordinate discharge planning in contract facilities.
• Educate potential customers about the benefits and appropriateness of hospice care.
• Ensure patients and their families have adequate knowledge and understanding of the hospice benefit.
• Process inquiries for all new referrals (internal and external).
• Facilitate a seamless transition from hospital to community by ordering medical supplies and DME, communicating with the field team, and documenting in the electronic medical record (EMR).
• Perform assessments of potential patients in accordance with Cleveland Clinic Hospice policies, procedures, and admission criteria alongside the physician.
• Conduct admission assessments for new admissions into contracted facilities and document information in the EMR within specified time frames.
• Plan patient care delivery in collaboration with other disciplines to provide comfort and symptom management.
• Perform daily assessments and coordinate the plan of care with inpatient staff for General Inpatients (GIP).
• Assess patients for safety risks, abuse, and neglect.
• Assess and document pain using appropriate pain scales and manage patient pain.
• Document changes in patient condition and the effectiveness/timeliness of care/interventions.
• Evaluate, update, and revise the plan of care to facilitate achievement of expected outcomes.
• Understand the action, usual dosage, indications, and side effects of administered medications.
• Attend interdisciplinary meetings as assigned.

Requirements

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• One year of current clinical experience as an RN.
• One year of experience in an acute med/surg or critical care setting.
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross.
• Knowledge of JCAHO and Medicare standards and regulations.
• Ability to travel to hospitals and community referral sources.

Preferred Qualifications

• Bachelor’s of Science in Nursing (BSN).

Physical Requirements

• Requires full range of body motion including handling and lifting packages, manual and finger dexterity, and eye-hand coordination.
• Occasionally lifts and carries items up to 50 pounds.
• Requires corrected vision and hearing to normal range.
• Requires standing and walking for extensive periods of time.

Schedule

This position works 8:00 AM to 4:30 PM with every 4th weekend and 2 holidays per year (1 summer and 1 winter).

Pay

The pay range for new hires is $30.41 to $46.38 per hour. A successful candidate’s actual compensation will be determined based on work history, experience, skill set, and education. The pay range does not include applicable pay practices such as shift differentials or overtime, nor does it include the value of Cleveland Clinic’s benefits package (e.g., healthcare, dental, vision, and retirement savings account contributions).
