About the role
Keep patients connected to the highest quality care as they transition from the hospital to home or a facility. Process inpatient referrals into hospice, ensure continuity of care and coordinate discharge planning in contract facilities. Educate potential customers about the benefits and appropriateness of hospice care. Enhance the quality of life for patients and their families.
Responsibilities
- Process inpatient referrals into hospice, ensure continuity of care and coordinate discharge planning in contract facilities.
- Actively educate potential customers about the benefits and appropriateness of hospice care.
- Ensure patients and their families have adequate knowledge and an understanding of the hospice benefit.
- Process inquiries for all new referrals (internal and external).
- Order medical supplies and DME, hand off communication with the field team and document in the electronic medical record.
- Perform admission assessments for new admissions into contracted facilities and document information in the EMR within specified time frame.
- Plan the delivery of patient care in collaboration with other disciplines to provide comfort and symptom management.
- Assess patient for safety risks, abuse and neglect.
- Document changes in patient condition and effectiveness/timeliness of care/interventions.
- Evaluate, update and revise the plan of care to facilitate achievement of expected outcomes.
- Know action, usual dosage, indications and side effects of medications administered.
- Attend interdisciplinary meetings as assigned.
Requirements
- Graduate from an accredited school of Professional Nursing
- Current state licensure as a Registered Nurse (RN)
- One year of current clinic experience as an RN
- One year of experience in an acute med/surg or critical care setting
- Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
- Knowledge of JCAHO and Medicare standards and regulations
Qualifications
- Ability to work irregular/varying hours as needed
- Ability to travel to hospitals and community referral sources
Skills
- Strong interpersonal and communication skills
- Excellent organizational and problem-solving abilities
- Proficiency in Microsoft Office applications

Pay
Minimum hourly: $30.41
Maximum hourly: $46.38
