As a Hospice Patient Care Coordinator (RN)

Collaborate with the Director of Clinical Services to develop and implement processes to ensure the efficient and effective achievement of patient/client outcomes in accordance with physician orders
Provide ongoing education to hospice employees including, but not limited to: regulations, guidelines, current standards of hospice and palliative care practice, Interim HealthCare Hospice polices and procedures, quality assessment and performance improvement (QAPI) programs, and each employee's role in each
Serve as an expert for the hospice admission process and perform admissions as assigned

Qualifications

• Graduate of an accredited school of nursing and licensed to practice as an RN in the state in which employed. License is active and in good standing.
• Two (2) years of experience as a Hospice RN required and at least one year of supervisory experience preferred

• Able to provide proof of valid driver's license and auto liability insurance in accordance with policy
