About the role
Empath Trustbridge Hospice is seeking a caring and motivated RN Hospice Case Manager to join their team in South Palm Beach County. Founded in 1978, Trustbridge Hospice provides compassionate, community-based care to families facing advanced illness.
Responsibilities
- Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
- Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
- Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
- Use clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
- Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust
Requirements
- Registered Nurse currently licensed in the State of Florida
- Current Basic Life Support Certification
- One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred
- Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
- Basic computer skills and EMR software experience a plus
Qualifications
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license
Skills
- Strong communication and interpersonal skills
- Ability to manage multiple tasks and prioritize effectively
- Knowledge of hospice care principles and practices

Pay
- Competitive pay based on experience and qualifications
Schedule
- Monday - Friday 8:30am - 5:00pm with a weekend requirement
