RN Hospice Case Manager
About the role
Cleveland Clinic’s hospice team includes physicians certified in hospice and palliative medicine, nurses, social workers, chaplains, home health aides and volunteers. This team works together to support patients wherever they call home.
Responsibilities
- Provide supervision, assessment, planning and direct patient care to hospice patients.
- Perform accurate and complete admissions assessments and re-assessments.
- Assess patients for safety risks, abuse and neglect.
- Assess, document and manage patient’s pain.
- Prioritize and document patient problems.
- Administer medications and treatments safely and accurately with a focus on patient comfort.
- Coordinate care for managing a caseload of patients.
- Document changes in patient condition and effectiveness and timeliness of care/interventions.
- Evaluate, update and revise plans of care.
- Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges.
- Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
Requirements
- Graduate from an accredited school of nursing.
- Current licensure as a Registered Nurse (RN) in the State of Ohio.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) or the American Red Cross.
- One year of current clinical experience as an RN.
- One year of experience in an acute medical-surgical or critical care setting.
- Valid state driver’s license.
- Proof of automobile insurance with 100,000/300,000 coverage.
Qualifications
- Bachelor of Science in Nursing (BSN).
- Two years of critical care and/or med/surg experience.
- Hospice and/or oncology experience.
Skills
Requires full range of body motion including handling and lifting packages, manual and finger dexterity, and eye-hand coordination. Requires standing and walking for extensive periods of time. Heavy work – Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.

- Short- and Long-term Disability Coverage – Short-term disability is available after one continuous year of regular full-time service; long-term disability begins after 90 days.

In addition to your contributions to the plan, Cleveland Clinic matches a portion of your contributions (50% on up to your first 6% in contributions).

Pay
Minimum Annual Salary: $73,840.00 Maximum Annual Salary: $115,648.00 The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.). The pay range does not include the value of Cleveland Clinic’s benefits package (e.g., healthcare, dental and vision benefits, retirement savings account contributions, etc.).
