Role Overview
This position is based in Sebring, FL and requires daily travel within Highlands and/or Hardee County. It is full-time, Monday thru Friday 8am - 5pm w/ participation in the on-call rotation.

Pay and Benefits
Salary Range: $80,000–$85,000 annually, with additional bonus opportunities. Final compensation may vary based on location, job-related knowledge, skills, and experience.

Nuts & Bolts of the Job
- Hospice RN Case Managers work Monday - Friday, 8AM - 5PM with a potential weekday on-call and Holiday coverage schedule
- Case Manage 15-18 patients, attend Interdisciplinary Team Meetings, contribute to team huddle calls, and receive updates on your patients daily
- Provide Case Management Assign Aide Visits
- Coordination of Care with Social Workers, Chaplains, Volunteers and Bereavement team
- Develop and nurture relationships with facility staff and leaders throughout the community

Downside of the Job
- Need to use personal vehicle if you drive less than 1000 miles a month fleet vehicle provided
- Caring for patients and families at end of life can create challenging situations
- Personal cell phone with monthly reimbursement
- On call rotation can get busy if the primary On Call RN is on PTO or the seat is temporarily vacant
Responsibilities
- Performs patient assessments and collaborates with the care team to develop and implement a plan of care
- Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so
- Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies
- Records pain/symptom management changes/outcomes as appropriate
- Teaches patients, families, and caregivers appropriate patient care techniques
- Regularly communicates with clinical manager and care team
- Performs on-call responsibilities and on-call services to patients/families as assigned
- Other duties as assigned
Qualifications
- Required: Current RN license in the state(s) where you will work
- Minimum 1 year of clinical RN experience in an acute healthcare setting
- Current CPR certification
- Preferred: Hospice or home health experience
- Strong communication and interpersonal skills
- Ability to work independently and manage multiple priorities
- Comfortable with daily travel between facilities
- Proficient in electronic medical record (EMR) systems
