RN Home Health - Phoenix, AZ - HUMJP00041208

Local or traveler position available for 13 weeks full-time, Monday through Friday, 8:00 AM to 5:00 PM (40 hours per week). 30 units per week in Home Health (HH) and Home Care Home Base (HCHB) required. This position is for the East Valley.

Responsibilities

• Provide skilled nursing care in a home health setting.
• Complete OASIS assessments and documentation in Homecare Homebase software.

Requirements

• Current state license as a Registered Nurse.
• Proof of current CPR certification.
• Current driver’s license, reliable transportation, and insurance.
• 1–2 years of experience as a Registered Nurse in a clinical care setting.
• 2+ years of home health experience.
• Experience with Homecare Homebase software and OASIS.
• HCHB and home health experience required.

Qualifications

• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program (required).
• Bachelor of Science in Nursing (preferred).
• CHPN Certification (preferred).
