About the role

We are seeking an experienced Registered Nurse (RN) for a home health position in Houma, LA. This is a 13-week assignment with the possibility of extension. Both local and travel nurses are welcome to apply.

Schedule

• Full-time, Monday–Friday, 8:00 AM – 5:00 PM
• 40 hours per week (30 productivity units)
• On-call required if requested; needs are subject to change based on staff and branch requirements

Responsibilities

• Provide skilled nursing care to patients in their homes, ensuring high-quality and compassionate healthcare.
• Conduct comprehensive assessments, including OASIS, to evaluate patient needs and track progress.
• Develop, implement, and update individualized care plans in collaboration with physicians and other healthcare professionals.
• Utilize Homecare Homebase (HCHB) software to document patient care accurately and efficiently.
• Educate patients and families on managing illnesses, treatments, and preventive healthcare practices.
• Maintain compliance with all healthcare regulations, documentation standards, and agency policies.
• Travel to patient homes, ensuring timely and reliable care delivery.

Requirements

• Current RN license or compact license in the state of LA
• 1–2 years of RN experience in a clinical care setting
• 2+ years of Home Health experience
• Homecare Homebase (HCHB) experience
• OASIS experience
• Proof of current CPR certification
• Current driver’s license, reliable transportation, and insurance

Qualifications

• Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program (Required)
• Bachelor of Science in Nursing (BSN) (Preferred)
• CHPN certification (Preferred)
