About the role

Provides direct patient care in a home health setting.

Responsibilities

• Administers medications and treatments as prescribed.
• Assesses patient’s condition and documents findings.
• Coordinates with healthcare providers to ensure continuity of care.
• Maintains accurate and timely documentation.
• Ensures compliance with regulatory standards and policies.

Requirements

• OASIS Experience Required.
• 30 units per week minimum requirement.
• On-call every 3rd weekend.

Qualifications

• Current state license as a Registered Nurse.
• CHPN Certification preferred.
• CPR certification required.
• Current driver’s license required.
• Reliable transportation and insurance required.
• 1-2 years experience as a Registered Nurse in a clinical care setting.
• 2+ years Home health experience.
