About the role

Hi we are seeking an experienced Registered Nurse (RN).

Responsibilities

• Provide skilled nursing care to patients in their homes, ensuring high-quality and compassionate healthcare.
• Conduct comprehensive assessments, including OASIS, to evaluate patient needs and track progress.
• Develop, implement, and update individualized care plans in collaboration with physicians and other healthcare professionals.
• Utilize Homecare Homebase (HCHB) software to document patient care accurately and efficiently.
• Educate patients and families on managing illnesses, treatments, and preventive healthcare practices.
• Maintain compliance with all healthcare regulations, documentation standards, and agency policies.
• Travel to patient homes, ensuring timely and reliable care delivery.

Qualifications

• Current RN license in the state of PA
• 1-2 years of RN experience in a clinical care setting
• 2+ years of Home Health experience
• Homecare Homebase (HCHB) experience
• OASIS experience
• Proof of current CPR certification
• Current driver's license, reliable transportation, and insurance

Education

• Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program - Required
• Bachelor of Science in Nursing (BSN) - Preferred
