About the role

13-week assignment in Grand Rapids, MI as a local or travel RN Home Health. Schedule: Monday–Friday, 8:00 AM–5:00 PM, 40 hours per week. On-call every other week (triage nurse handles calls). Weekend RN covers 30 units in Homecare Homebase (HCHB). OASIS and home-health experience required. No guaranteed hours.

Requirements

• 2+ years of Home Health experience
• Active RN license in the practicing state
• CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program (required)
• Bachelor of Science in Nursing (preferred)
• CHPN certification (preferred)

• Strong clinical judgment and patient assessment skills
