About the role

We are seeking an experienced Registered Nurse (RN) to join our team serving patients throughout Rutherford County, TN. This role is responsible for providing skilled nursing care, completing comprehensive patient assessments, developing and implementing individualized care plans, and delivering compassionate, high-quality care in the home setting. The ideal candidate will have strong clinical judgment, excellent communication skills, and a commitment to improving patient outcomes through education, coordination, and continuity of care.

Schedule

Monday-Friday, 8:00 AM - 4:30 PM

• Productivity expectation: 30 points per week
• On-call requirement: Minimum of 1 weekend per month with LPN backup support
• Mileage reimbursement provided between patient visits

Pay

$51-$53/hour

Responsibilities

• Perform comprehensive patient assessments and develop individualized nursing care plans
• Provide skilled nursing care, treatment, rehabilitation support, and disease management in the home setting
• Identify and address patients' physical, psychosocial, safety, and comfort needs
• Administer medications, treatments, and nursing interventions within scope of practice
• Monitor patient progress, evaluate outcomes, and update care plans as needed
• Observe, document, and communicate changes in patient condition to physicians and clinical leadership
• Educate patients, families, and caregivers on health maintenance, disease management, and treatment plans
• Maintain accurate and timely clinical documentation in accordance with agency, state, and regulatory requirements
• Collaborate with physicians, therapists, and interdisciplinary team members to ensure coordinated patient care
• Supervise and support auxiliary nursing personnel and promote adherence to the nursing process and best practices

Requirements

• Active Tennessee Registered Nurse (RN) License
• Current BLS Certification
• Minimum 3 years of RN experience
• Minimum 1 year of Home Health RN experience
• Strong clinical assessment and care planning skills
• Valid driver's license and reliable transportation
• Ability to work independently while managing a home-based patient caseload

• Experience with home health documentation systems and electronic medical records (EMR)
• Strong knowledge of regulatory and compliance standards
