RN - Home Health

Pride Health is hiring a Registered Nurse - Home Health for a 15-week contract assignment supporting a leading healthcare organization in Houghton, MI.

Responsibilities

• Provide skilled nursing care to home health patients, including assessments, care coordination, and management of individualized patient care plans.
• Perform skilled nursing visits including wound care, dressing changes, medication teaching, medication setup, and other patient care needs.
• Complete patient admissions, recertifications, and discharges while maintaining accurate clinical documentation.
• Provide education and support to patients and families to promote safe and effective care in the home setting.

Requirements

• Active Registered Nurse (RN) license.
• Home health or home care nursing experience required.
• Experience with PICC lines and wound vacs preferred.

Schedule

• 15-week contract assignment.
• Start Date: ASAP.
• Monday-Friday, 8:00 AM - 4:30 PM (8-hour shifts / 40 hours per week).
• On-Call Requirement: Once monthly weekend rotation and 3 weekdays per month.

Coverage Area

• Houghton County, partial Keweenaw County, and partial Baraga County.
• Mileage reimbursement provided between patient visits.

Pay

$53.00-$57.00/hour
