RN - Home Health

Pride Health is hiring a Registered Nurse - Home Health for a contract opportunity supporting a leading healthcare organization in Indianapolis, IN.

Responsibilities

• Complete skilled home health nursing visits, including OASIS Start of Care (SOC), recertifications, and discharge assessments.
• Perform comprehensive patient assessments, medication management, and wound care in the home setting.
• Maintain accurate and timely documentation using Homecare Homebase (HCHB) and other home health EMR systems.
• Coordinate care plans with the healthcare team while providing compassionate, patient-centered care.

Requirements

• Active Registered Nurse (RN) license.
• Previous Home Health nursing experience required.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong communication skills and a positive, patient-focused attitude.

Schedule

• Monday-Friday, 8:00 AM–4:30 PM
• 40 hours per week
• Approximately 14-week contract
• Start date: ASAP
• No on-call requirement

Pay

$53.00–$57.00 per hour

Mileage reimbursed between patient visits.
