Pride Health is hiring a Registered Nurse - Home Health for a contract opportunity supporting a leading healthcare organization in Indianapolis, IN. Responsibilities Include: • Complete skilled home health nursing visits, including OASIS Start of Care (SOC), recertifications, and discharge assessments. • Perform comprehensive patient assessments, medication management, and wound care in the home setting. • Maintain accurate and timely documentation using Homecare Homebase (HCHB) and other home health EMR systems. • Coordinate care plans with the healthcare team while providing compassionate, patient-centered care. Qualifications: • Active Registered Nurse (RN) license. • Previous Home Health nursing experience required. • Experience with Homecare Homebase (HCHB) preferred. • Strong communication skills and a positive, patient-focused attitude. Important Details: • Employment Type: Approximately 14-Week Contract • Start Date: ASAP • Schedule/Shift: Monday-Friday | 8:00 AM-4:30 PM | 40 hours/week • On-Call Requirement: None • Coverage Area: Indianapolis, IN • Mileage Reimbursement: Mileage reimbursed between patient visits • Pay Range: $53.00-$57.00 per hour Limited openings!
