Position Overview
Seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients throughout Lawrence and Morgan Counties. This role is ideal for a nurse who thrives in an independent field-based environment and is committed to delivering high-quality, patient-centered care. The RN will manage a caseload of patients, perform clinical assessments, coordinate care, and provide specialized nursing services, including wound care and IV therapy.
Schedule
Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 40 hours
- On-Call/Weekend Requirement: As requested by the agency
- Coverage Area: Lawrence and Morgan Counties
- Mileage Reimbursement: Yes, reimbursed between patient visits (from first patient to last patient)
Pay
$51.00 - $53.00 per hour
Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans.
- Perform comprehensive patient assessments and ongoing evaluations.
- Deliver specialized nursing services, including wound care and IV therapy.
- Develop, implement, and update patient care plans based on clinical findings and patient goals.
- Monitor patient progress, identify changes in condition, and communicate updates to physicians and care team members.
- Educate patients and caregivers regarding disease management, medications, treatment plans, and home care needs.
- Coordinate services with physicians, therapists, social workers, and other interdisciplinary team members.
- Complete accurate and timely clinical documentation in compliance with agency, state, and federal requirements.
- Manage an assigned patient caseload while maintaining productivity expectations.
- Participate in on-call and weekend coverage when requested by the agency.
Requirements
- Active Alabama Registered Nurse (RN) license in good standing.
- Experience providing wound care and IV therapy.
- Strong clinical assessment and critical thinking skills.
- Ability to work independently in a home health environment.
- Excellent communication, organization, and time management skills.
- Valid driver’s license and reliable transportation.
Preferred Qualifications
- Previous home health experience.
- Experience with Homecare Homebase (HCHB).
- Knowledge of home health regulations, documentation standards, and best practices.
- Strong patient advocacy and relationship-building skills.
- Ability to effectively manage a caseload while delivering exceptional patient care.
