Hours & Pay
Weekly hour guarantee: 40 hours/week. Pay range: $52.00–$54.00/hour (based on experience, expertise, credentialing, and education). Mileage reimbursed between visits at the applicable mileage rate.
About the role
Conduct comprehensive in-home patient assessments, develop and update individualized care plans, and monitor patients for changes in condition or complications. Provide skilled nursing care, including medication administration, wound care, post-hospital or post-surgical care, IV therapy when ordered, and chronic disease management. Educate patients and caregivers on medications, disease management, self-care, infection prevention, and home safety while advocating for patient needs and promoting independence. Coordinate care with physicians and interdisciplinary team members while completing accurate, timely clinical documentation and maintaining Medicare, CMS, and agency compliance.
Requirements
- Active, Registered Nurse (RN) license
- Basic Life Support (BLS) Certification
- 1+ years of experience

- HomeCare HomeBase (PointCare) EMR experience
