About the role
Pride Health is hiring a RN - Home Health to support our client's healthcare facility in Wilson, NC.
Schedule
- Monday-Friday | 8:00 AM-4:30 PM
- 36 Guaranteed Hours/Week
- On-Call/Weekend: No
- Location: Wilson, NC; Coverage areas include Nash, Edgecombe, and Wilson Counties
- Mileage reimbursed between visits per contract company
- Job Type: Contract
- Contract Length: 8 Weeks
Pay
$52-$54 per hour, offered based on experience, expertise, credentialing, and education.
Responsibilities
- Manage and coordinate patient care as an RN Case Manager
- Assess patient needs and develop individualized plans of care
- Coordinate services and communicate with physicians and interdisciplinary healthcare teams
- Complete accurate and timely patient documentation and follow-up
Requirements
- Active RN license
- BLS certification
- 1+ years of experience
- Home Health experience preferred
- HCHB experience preferred
