About the role

Home Care RN serving PACE participants in the Providence/East Providence area. Community-based nursing as part of the interdisciplinary team (IDT): safety and acute-care assessments, medication reconciliation, transitions of care, care planning, and quarterly observation/oversight of home care CNAs.

Requirements

• RI RN license
• CPR certification
• Driver's license
• Reliable transportation

Qualifications

• ADN required
• BSN preferred
• Geriatric/home care experience preferred but not required

Schedule

Weekday days with rotating weekend/holiday coverage as scheduled (24‑hour program coverage; exact rotation cadence being confirmed).
