Position Overview

We are seeking compassionate and dedicated LPN or Registered Nurse's (RN) to join our team. You will be responsible for assessing, planning, and overseeing the implementation of personalized care plans for our clients, ensuring they receive the highest quality of in-home care.

Responsibilities

• Client Assessments: Conduct comprehensive evaluations of clients' physical, mental, and psychosocial needs to develop individualized care plans.
• Care Plan Development: Collaborate with clients, families, and the care team to create and update personalized care plans that address identified needs.
• Care Coordination: Monitor and coordinate services to ensure adherence to care plans, making adjustments as necessary to meet changing client needs.
• Staff Training and Support: Provide guidance and mentorship to caregivers, ensuring they are well-informed about care plans and best practices.
• Documentation: Maintain accurate and timely records of assessments, care plans, and any changes in client conditions.
• Compliance: Ensure all care practices comply with federal, state, and local regulations, as well as company policies.

Qualifications

• Active Nurse license in the state of South Carolina.
• Minimum of one year of experience in long-term care or home health care settings.
• Strong assessment, critical thinking, and problem-solving skills.
• Excellent communication and interpersonal abilities, with a compassionate approach to client care.
• Ability to work independently and as part of a collaborative team.

Pay

• Pay is per completed client assessment, not by the hour.
• $40 for LPN initial assessments / $35 for supervisory visits. Plus mileage $0.57 per mile.
• $50 for RN initial assessments / $45 for supervisory visits. Plus mileage $0.57 per mile.

• Employment Type: PRN (Contract Work)
• Flexible scheduling options to promote work-life balance.
• Location: Columbia, SC, flexible work locations: your home, client's residence. Our home office is in Spartanburg, SC.
