About the role

Provide PRN (as-needed) home care services as a Registered Nurse (RN) in the Jefferson area.

Responsibilities

• Deliver skilled nursing care in patients' homes, including assessments, wound care, and medication administration.
• Educate patients and families on self-care strategies.
• Develop and update individualized care plans.
• Coordinate with physicians and interdisciplinary teams to ensure comprehensive care.
• Accurately document visits and monitor patient condition for any changes.

Requirements

• Current RN licensure.
• Strong clinical skills in home health care, including wound care and medication management.
• Excellent communication and patient education abilities.
• Ability to collaborate effectively with healthcare teams.
• Compassionate, detail-oriented, and adaptable to patient needs.
