About the role
Own your participants' care in the community as a core member of PACE‑RI's interdisciplinary team.
- Assess safety and acute care needs in the home.
- Reconcile medications and manage safe transitions of care.
- Build care plans alongside the primary care provider and social worker.
- Triage participant and family calls.
- Coordinate home services with the scheduler.
- Supervise and teach CNAs delivering personal care.
Requirements
- Active Rhode Island RN license.
- Current CPR certification.
- Associate degree (BSN preferred).
- Driver's license and reliable transportation.
- Experience preferred but not required; new graduates welcome.
