About the Role

Responsible for providing quality home nursing services through effective case management, adhering to community health nursing standards and agency policies.

Responsibilities

• Deliver personalized home nursing care to patients
• Manage patient cases in accordance with policies and standards
• Collaborate with healthcare teams to ensure quality care
• Maintain proper documentation and compliance with licensing requirements

Requirements

• Graduate of an accredited registered nursing program
• Licensed as a Registered Nurse in the state
• Minimum of two years' Medical/Surgical nursing experience
• Valid driver's license and auto liability insurance

Preferred Qualifications

• Bachelor of Science in Nursing
• Home health care experience
• Additional relevant certifications
