RN - Full-Time Hospice

Role: The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care by evaluating patient needs, developing and implementing personalized care plans, and collaborating with patients, families, and healthcare professionals. Key Responsibilities Conduct initial nursing evaluations to determine hospice eligibility within 48 hours of referral.Reassess patient needs regularly, at least every 15 days, addressing physical, psychosocial, and environmental factors.Develop and coordinate individualized care plans, documenting goals, interventions, and responses.Collaborate with the interdisciplinary team, including physicians and family members, to ensure quality care.Educate and supervise patients and families on self-care techniques when appropriate.Maintain accurate clinical documentation of patient conditions and progress.Provide care adaptable to diverse home environments, respecting cultural and individual differences. Qualifications & Skills Current, unrestricted RN licensure in the practicing state.Valid driver’s license, insurance, and reliable transportation or access to public transit.Preferred: 1+ years of clinical experience and CPR certification (or ability to obtain within 90 days).
