RN Full-time Hospice

About the Role

As a Registered Nurse Case Manager at Elite Hospice, part of LHC Group and the Optum family, you will assume full nursing responsibility for delivering the Plan of Care for each hospice patient. You will continuously evaluate personal and professional performance to enhance productivity and quality of care. Working collaboratively with other professionals, you will develop and implement treatment strategies that promote physical, psychosocial, emotional, and spiritual well-being. This role requires familiarity with the needs of patients and families facing death and dying.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify and reassess the patient’s/family’s physical, psychosocial, and environmental needs at least every 15 days.
• Initiate and coordinate the Plan of Care, documenting problems, goals, interventions, and patient/family responses.
• Collaborate with the patient/family, attending physician, and Interdisciplinary Group (IDG) to provide daily care.
• Instruct and supervise patients/families in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient’s condition.
• Function effectively in any home situation, regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Preferred Qualifications

• 1+ years of clinical experience.
• Current CPR certification or ability to complete within 90 days of hire.

Pay

The salary for this role ranges from $71,640 to $107,640 annually, based on full-time employment. Pay is determined by factors including local labor markets, education, work experience, and certifications.
