RN (Full/Part Time) - Transitional Care Unit

About the role

We are seeking licensed Registered Nurses (RNs) to provide comprehensive, whole-person care in a transitional care setting.

Responsibilities

• Collaborating with an interdisciplinary team to deliver high-quality patient care.
• Managing medical complexities such as wounds, IV therapy, dialysis, oxygen therapy, and TPN in a home-like environment.
• Participating in shared governance to develop and uphold practice standards.

Qualifications

• Licensed as a Registered Nurse in the state.
• Current BLS certification from the American Heart Association.
• Preferred: Bachelor's degree in Nursing, food handler’s permit, and 1+ year of Med-Surg experience.
• Experience supervising CNAs is a plus.
